- The complete personnel file for employee, JOHN DOE, Social Security ending number XXXX, together with his payroll file.
- All agreements for employment between BROWARD COUNTY SCHOOLBOARD and JOHN DOE.
- Any and all documents evidencing the hours worked by JOHN DOE and the rate of pay he receives from the date of his employment through the present, including but not limited to W-2’s and 1099’s.
- Any and all documents pertaining to benefits received by JOHN DOE from the date of his employment to the present, including but not limited to health insurance, life insurance, and disability insurance.
- Any and all documents evidencing expense reimbursements, if any made to JOHN DOE from the date of his employment to the present.
- Any and all documents evidencing any bonuses and commissions received by JOHN DOE from the date of his employment to the present.
- Any and all documents evidencing any other benefits or compensation has been paid to JOHN DOE from the date of his employment to the present, including but not limited to stock options.
- All records in support of the accumulation and/or use of sick days/hours and vacation days/hours by JOHN DOE from the date of his employment to the present and policies regarding same.
- All pension records for JOHN DOE including all records regarding his pension accounts of any form; all deferred compensation accounts, 401K plans, 403B’s, SEP Plans, or otherwise from the date of his employment to the present.
- The Summary Plan Description of any pension and other retirement benefits relative to JOHN DOE from the date of his employment to the present.
- Any and all other records regarding retirement benefits/deferred compensation/pension which have accrued to the benefit of JOHN DOE.
- All documents in support of advances and/or loans made by your company to JOHN DOE from the date of his employment to the present, inclusive of all documents evidencing repayment of same in whole or in part.
