AGREED FINAL JUDGMENT OF DISSOLUTION OF MARRIAGE WITH DEPENDENT OR MINOR CHILD(REN) AND PROPERTY AND ORDER UPON REPORT OF GENERAL MAGISTRATE
- The Court has jurisdiction over the subject matter and the parties.
- At least one party has been a resident of the State of Florida for more than 6 months immediately before filing the Petition for Dissolution of Marriage.
- The marriage between the parties is irretrievably broken. Therefore, the marriage between the parties is dissolved, and the parties are restored to the status of being single.
- An Agreed Report of General Magistrate/ Parenting Plan incorporating the parties Settlement Agreement/Parenting Plan is ratified and the parties shall abide by the Mediation Agreement/Parenting Plan.
- Commencing on XXXXXX XX, 20__, the Husband shall be responsible to pay child support in the amount of $494.84 per month until XXXXX XX, 20__ when the oldest child reaches 18 and/or until the child graduates high school. The Husband’s child support obligation for the remaining minor child shall be $311.04 per month or until such time as the minor child dies, joins the armed forces, or otherwise becomes emancipated, whichever first occurs. A child support guideline worksheet is attached hereto as Exhibit “2” and Exhibit “3”.
- The court finds it is in the children’s best interest that all child support be paid directly to the Florida State Disbursement unit, PO Box 8500, Tallahassee, FL, 32314-8500 through Income Withholding Order.

The payor shall list the case number and the parties’ names on all checks. The (recipient) shall be responsible for setting up an account with the child support depository.
- Health/Dental and Vision Insurance. The Wife shall be required to continue to maintain health, dental, and vision insurance for the parties’ minor children, so long as reasonable in cost and accessible to the children.
- Reasonable and necessary uninsured medical, dental, prescription drug and orthodontic costs for the minor children shall be assessed with the Husband responsible for 24% and the Mother responsible for 76% pursuant to the child support guidelines worksheet (attached hereto as Exhibit “2”)
- The wife’s former name is hereby restored to: “XXXXXXXXX”.
- The Court reserves jurisdiction to modify and enforce this Final Judgment.
