MANNER AND METHOD OF PAYMENT

 

  1. The Payor effective XXXXX XX, 20__ and every 10th of the month thereafter shall pay the sum of $200.00 per month until the child support arrears in the amount of $13,300.00 is paid in full. The final payment shall due on XXXXXXX XX, 20__.  The final payment shall be $100.00.   
  2. Any payment which becomes due prior to the date of entry of this Order shall be made in accordance with the payment provisions of the previous Order, and in the absence of such Order, payment shall be made directly to the Payee.

 

  1. Payments shall be made payable to CLERK OF THE CIRCUIT COURT and forwarded to:

  1. The Support Enforcement Division shall collect a flat fee in the sum of Five Dollars and Twenty-Five Cents ($5.25) for receiving, recording, reporting or handling alimony and child support payments. This flat fee shall be reduced in any case in which said fee would result in a charge to any party of an  amount greater than four percent (4%) of the alimony or child support payments such party is obligated to pay, except that no fee shall be less than One Dollar and Twenty-Five Cents ($1.25).  The Respondent/Payor shall be responsible for the fee.

 

  1. The parties affected by this Order shall immediately inform the Support Enforcement Division of any change of name, address, employment, or source of income.
