I, JIMMY DOEE, am the biological and natural Father of VIOLET, born on XXXXX, 20__ and TAYLOR, born on XXXXX, 20__ to JANE DOE and do hereby consent to the custody of the minor children, to the Petitioner, JOHN DOE, Paternal Grandfather with full knowledge of the legal effect of the custody proceeding.
  2. I understand my legal rights as a parent and I understand that I do not have to sign this consent. I acknowledge that this consent is being given knowingly, freely and voluntarily.
  3. I waive any further notice of the custody proceeding.
  4. I have not been interviewed about this proceeding pursuant to a preliminary home study.
