I, JANE DOE, am the biological and natural Mother of XXXXX, born on XXXXXX, 20__ and XXXX on XXXXX, 20__, and do hereby consent to the custody of the minor children, to the Petitioner, JIMMY DOE, Paternal Grandfather with full knowledge of the legal effect of the custody proceeding.

I hereby acknowledge that JOHN DOE is the biological and natural Father of both minor children, Violet, born to me on December 26, 20__ and TPR, born to me on March 22, 20__.

I understand my legal rights as a parent and I understand that I do not have to sign this consent. I acknowledge that this consent is being given knowingly, freely and voluntarily.

I waive any further notice of the custody proceeding.

I have not been interviewed about this proceeding pursuant to a preliminary home study.
