FINAL JUDGMENT OF PATERNITY WITH PARENTING PLAN
THIS CAUSE having come before the Court on XXXX XX, 20__ on the Father’s Verified Complaint to Determine Paternity, Shared Parental Responsibility, Majority Sharing, Parenting Plan, Child Support and Other Relief. The Petitioner, the Petitioner’s attorney, Larry D. Schott, Esquire, and the Respondent were present.
ACCORDINGLY, it is ORDERED and ADJUDGED:
- This is an action for paternity determination and support pursuant to Chapter 742 of the Florida Statutes, Chapter 61 of the Florida Statutes, and other applicable Law and Rules
- The Court has jurisdiction over the parties hereto and the subject matter herein.
- The court has jurisdiction to determine child support, timesharing, and
parental responsibility with regard to the minor child, JACK KING., born on XXXXX XX, 20__.
- The parties were not married at the time of the child’s conception or at any subsequent time.
- The Father’s Complaint to Determine Paternity, Shared Parental Responsibility, Majority Sharing, Parenting Plan, Child Support and Other Relief is hereby granted. The parties have resolved all issues in this matter and have reached an agreement that is in the child’s best interests.
- The parties agree and the Court affirms that Petitioner, JACK KING, is the natural and biological father of Jack King Jr., born on XXXX XX, 20__ in Broward County, Florida.
PARENTING PLAN
- The parties agree that they shall have shared parental responsibility of the minor child, Jack King, Jr., born on XXXXX XX, 20__.
- The parties agree that the Petitioner/Father is permitted to relocate to Honolulu, Hawaii on or about XXXX XX, 20__ with the minor child, Jack King, Jr., born on XXXXXX XX, 20__, who shall permanently reside with him in his custody and attend school in Honolulu, Hawaii.
- All school records shall be given to the Father and transferred to the new school the child will be attending in Hawaii.
- The Mother shall have the following timesharing with the minor child as follows in Florida annually:
- One (1) week during Winter break and the entire summer break except that the child will be returned to the Father one week before school resumes.
- One weekend per month in Hawaii if the Mother is present in Hawaii.
- The Mother shall be solely responsible for any and all transportation costs to exercise her timesharing during the Winter Break and Summer Break
- The Father’s child support obligation in Case Number: XXXXXX is hereby abated.
- The Petition for Modification of Support in Case Number: XXXXXX is hereby withdrawn.
- Neither party shall have a child support obligation to the other party. Each party shall take care of the child while in their care.
- This Court retains jurisdiction of the parties hereto and the subject matter hereof for all purposes, including without limitation, for the purposes of enforcing and modifying the provisions of this Final Judgment of Paternity with Parenting Plan.
