this his Motion for Continuance of XXXXXXX XX, 20__ Hearing and states as follows:

  2. The undersigned received a Notice of Hearing which was not coordinated with the undersigned.
  3. The undersigned is out of State from XXXXXX XX, 20__ through XXXXXX XX, 20__; XXXXXXX XX, 20__ and XXXXX XX, 20__; and XXXXXXX XX, 20__ through XXXXXX XX, 20__ and is requesting a continuance of the hearing.
